can Home Care Services do for you? |
- Home Care Services can
help you live with independence and dignity in your
own home, near family and friends
- Home Care Services can
delay admission, or offer an alternative, to long-term
care placement

Eligiblity |
- You must reside in
Regional Health Authority #6
- You must have a Personal
Health Number (PHN) or be in the process of obtaining
one
- Need service, and not
have such services available from family and friends
- Age and income DO NOT
affect eligibilty

What services does Home
Care provide? |
- Assessment and
Coordination - Staff will set up services and can
arrange to make referrals to other agencies that can
assist you.
- Nursing - Nurses
perform a variety of nursing procedures as prescribed
by a physician. Nurses will teach self-care as well as
instruct families on how to provide care. Nurses also
provide personal care to clients requiring
professional help such as bathing, dressing, nail
care, feeding, and toileting.
- Meals on Wheels -
A hot nutritious meal is delivered to your home. Staff
also can provide special diets to clients as prescribed
by a physician.
- Homemaking -
Trained Home Health Care Aides can provide both
Personal and Respite care.
- Volunteer Services
- Volunteers play an important role in our program by
providing Friendly Visits, Telephone Reassurance,
Surveillance, Meal Delivery, and Palliative Care.
Teenagers also visit Senior Citizens in our
Adopt-A-Grandparent program.
- Palliative Care -
Client Case Managers, Nurses, Home Health Aides and
Volunteers are part of the team who assist in
accessing/providing services required to meet the
needs of a person living with a terminal illness.
